You'll be making a real difference to your patients every single day - deliver outstanding community care as a valued member of the Community Service. Discharge Home To Assess service runs 8am to 6pm Monday to Friday and 9am to 5pm at weekends. The post holder will be expected to contribute to the weekend working Rota. The Trust is currently going through a transformation, therefore there may be some changes, this can be discussed in further either at interview or by contacting us beforehand. The service covers both North and East Hertfordshire and the post holder may be expected at times to cover across localities.
