Please note South Warwickshire and Worcestershire Mind has recently merged with Coventry and Warwickshire Mind. You will be employed by Coventry and Warwickshire Mind but working for South Warwickshire and Worcestershire Mind. Key responsibilities Empower individuals: Work directly with clients to overcome early signs of mental health distress, such as anxiety, depression, and stress. Deliver tailored support: Utilise a facilitated self-help model to guide clients through personalised support plans. Collaborate with partners : Work closely with GPs and other healthcare professionals to ensure seamless care. Manage caseload: Efficiently manage a caseload of Active Monitoring clients. Contribute to service development: Provide valuable insights to enhance the Active Monitoring program. Essential requirements Experience: 12 months of experience working face-to-face with individuals experiencing mental health difficulties. Qualifications: Level 3 in Health & Social Care or equivalent. Knowledge: Strong understanding of mental health, social care, and risk assessment. Skills: Excellent communication, interpersonal, and organisational skills. Passion: A genuine commitment to improving mental health and well-being.
