The position is within the Older persons with physical disability team which is a community based team working with Adults over 18 years of age with a range of physical and Mental health needs.This is a dynamic post that requires the post holder to work closely with multi-disciplinary teams/agencies in addressing a range of care/support issues in the promotion of independence and improving the health and wellbeing of the population served. As a Care Act Assessor you will complete holistic assessments for individuals within the Stafford area in order to identify needs and promote independence. You will identify appropriate services to meet the needs of individuals and complete reviews to ensure outcomes are met. You will be working within a multi disciplinary team including external partnership agencies to support adults in the Stafford area. The post holder will be expected to access Social care and health records to gather information, therefore it is important for the post holder to be able to use a computer and have an understanding of the importance of good recording. You will require effective communication and written skills and be willing to work as part of a team to promote best outcomes for individuals. You will be required to be self-motivated and organised in order to manage any competing demands and be provided with all relevant training and support in the role. Midlands Partnership Foundation Trust is dedicated to ongoing Social Care development and you will have access to the Social Work learning academy in order to promote professional development. You will also receive the support of your team and your line manager who will provide regular supervision both on a formal and informal basis. Social Work Learning Academy (SWLA) On appointment, you will be enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration.
