Alma Medical Centre is looking to expand its current dynamic team and is actively recruiting a Healthcare Assistant . This is an exciting opportunity for an enthusiastic and motivated individual to join our friendly team at Alma Medical Centre in Stockton. Applicants should have excellent communication skills and be passionate about high quality patient care. The successful candidate will work at Alma Medical Centre, Stockton providing healthcare to our patients with a focus on elderly and frail patients. This role will involve carrying out home visits for patient assessments, health checks, medication compliance and social interactions. Therefore, a current driving license is essential. The focus of this role is to provide frailty health checks, phlebotomy, general health care, and support to our elderly population to improve their health and wellbeing. Providing general health check-ups and creation of personalised care and support planning is a series of facilitated conversations in which the person, or those who know them well, actively participates to explore the management of their health and well-being within the context of their whole life and family situation. You will work closely with our GPs, Health Professionals, Practice Staff, and wider multi-agency teams to provide advice on lifestyle issues, assist and support patients in changing their unhealthy behaviours and refer appropriately.
