Your primary responsibilities will include supporting insulin treatment, gathering and providing feedback on Blood Glucose Levels, training patients on medication, insulins, and equipment, downloading patient BGLs and information, setting up CGMs for DSNs to review, participating in DESMOND/Patient T2DM Education, and collecting initial information calls to provide comprehensive information to DSNs for informed triage. DCT for the Cambridge Team will be require to carry out home visits at the request of the DSN. Peterborough DCT will be expected to support the Consultant clinics running from the Healthy Living Centre. The role includes promoting a better understanding of Diabetes for patients, their families and carers, and other agencies involved with the patient through education, support and counselling.
