The successful candidate will work alongside the Neurovascular Consultants and CNS to deliver this specialist service. Demonstrate knowledge of the diagnosis, treatment and complications associated with these conditions. Provide specialist clinical assessment of patients, offer symptom management and early identification of problems. Provide advice and support for practical aspects of life e.g work, driving. Offer emotional support to patients and families.The post holder will provide education and consult widely with professional colleagues while promoting a progressive attitude to the continual improvement of patient care through research, audit, development and innovation as part of the multidisciplinary.The successful candidate will need to work as part of a multidisciplinary team, providing nursing leadership in adult neurovascular disorders, within Cardiff and Vale. Act as a source of specialist knowledge and assist in meeting the information needs of individuals, their families, carers and staff members both directly and indirectly through information provision and signposting. Individualised assessments and discussions with patients and families during inpatient phases of recovery. If interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post. Post is secondment for 12 months to meet needs of the service You will be required to act as a leader and role model in adult neurovascular disorders, setting standards & expectations of quality of care. You will be able to offer support and counselling at the time of referral, diagnosis or change in needs of the adults and families you are supporting. You will need to ensure that the information is displayed in all necessary hospital clinics about the MDUK, Wales Neuromuscular Network and the Nurse Specialist for Primary Muscle Disease & how they can be contacted. Initiate and promote national networking & sharing of expertise and knowledge. Ensure all care given will satisfy all relevant legal, professional and clinical governance responsibilities. Develop, implement, monitor and audit clinical guidelines/protocols for the management of patients, related aspects of care and lead implementation/evaluation. Share this knowledge at a local, national and international level as appropriate, to ensure continual improvements in patients care. Ensure registration with the Nursing and Midwifery Council is maintained. Be aware and adhere to the code of professional conduct for the nurse, midwife and health visitor. Adhere to mandatory training and Revalidation requirements. Report and record all incidents according to Health Board process of complaints and incidents, and participate in official investigations when required.
