Please feel free to contact Sheruba Draviaraj (PCN manager) on sdraviaraj@nhs.net OR Dr Ollie Hart (PCN Clinical Director)oliver.hart@nhs.net if you have any questions about the role Please take a look at the services we offer our patients- www.heeleyplus.co.uk We anticipate this role will include (by mutual negotiation): -4-6 sessions / week -option for flexible working hours (late/early starts, evening sessions) -Good supervision and mentorship with senior PCN GPs and managers -Opportunity to join innovate approaches to Metabolic health and Chronic pain, including group consultation, lifestyle interventions, peer support. -Working as part of our PCN MDTs -Opportunity to develop leadership skills (work alongside PCN Clinical Director) -Time for personal development, and non- patient facing learning -Mix of core General Practice clinics in practices and more innovative services (as above) About us In accordance with the agreed PCN timetable, the post-holder will undertake a variety of duties, including: Conducting surgery and telephone consultations, addressing patient queries, and visiting patients at home. Checking, signing, and authorising prescriptions, while managing queries, paperwork, and correspondence in a timely manner. Making professional, autonomous decisions regarding presenting problems, whether self-referred or referred by other health care workers. Assessing the healthcare needs of patients with undifferentiated and undiagnosed issues. Screening patients for disease risk factors and early signs of illness. Developing care plans in consultation with patients, in line with current disease management protocols. Providing counselling and health education. Admitting or discharging patients from the caseload and referring them to other care providers as appropriate. Recording clear and contemporaneous consultation notes to agreed standards. Collecting data for audit purposes. Attending MDT meetings Participation in education/teaching of wider MDT teams including medical and non medical teams Compiling and issuing computer-generated acute and repeat prescriptions, avoiding hand-written prescriptions whenever possible. Prescribing in accordance with the Practice prescribing formulary or generically, as clinically appropriate. Undertaking emergency patient visits during surgery hours, if clinically indicated. Participating in on-call sessions and home visiting sessions as allocated in the practice rota. Other Responsibilities within the Organisation Adhering to all relevant Practice policies and guidelines, including those related to prescribing, confidentiality, data protection, and health and safety. Committing to lifelong learning and audit to ensure evidence-based best practices. Contributing to evaluation, audit, and clinical standard setting within the organisation.
