We are very excited to establish this new post for a Neuro-navigator in Enfield. You will support patients with complex neuro rehabilitation needs as they move from acute services through to in-patient and community services, and also in supporting those with longer term neuro conditions . You will have an active caseload to support you to retain your clinical skills. The role involves working in close partnership with acute services, specialist neuro rehab services, local rehab and community services in identifying the most appropriate patient pathway utilising inpatient rehab beds and community provision. You will work alongside Adult Community Therapy colleagues and have peer support from other NCL neuro navigators in the North Central London Sector. You will be an integral member of the Adult Community Neurological Therapy Team based at St Michaels primary care centre EN1. As a new post and a developing service, there will be ample opportunity for service development, and clinical leadership.There is an established in-service training programme. Leadership and formal training opportunities are also available. This post is for 32 hours per week Mon- Friday service and we are happy to discuss flexible working patterns and would welcome you to attend for an informal visit. With ongoing developments in community services there is the possibility of weekend working in the future. The post holder will lead the management of Enfield patients with complex neuro-rehabilitation needs throughout the neurological rehabilitation pathway from acute care, specialist inpatient neurorehabilitation and into the community by carrying an active caseload and by implementing and review of a collaborative pathway To support the disability management of patients with a degenerative neurological condition. As disease progresses to support patients and their support network with accessing appropriate support, CHC applications and associated resources. Provide expert assessment as an autonomous practitioner of patients being recommended for treatment to specialist rehabilitation units and to ensure that patients with specialist neuro rehabilitation needs are referred to the most appropriate service to meet their needs To monitor patients in terms of their rehabilitation needs who are in specialist rehabilitation settings in liaison with North Central London ICB To work in partnership with other health and social care providers, promoting an integrated approach to the management of complex neurological conditions in Enfield Provide a case management approach for complex cases, acting as a central point of contact for the multidisciplinary team including the patient and their family.
