You will be based in one practice Rother House Medical Centre. We are a dynamic general practice in Stratford upon Avon looking for someone to join our amazing team looking after 16500 patients. We are looking for someone with a can-do attitude who is passionate about patient care and values everything primary care can offer. We put patient care at the centre of everything we do and work closely as a team to provide the best possible care within the resources available to us . The candidate should be able to demonstrate that they can Assess , diagnose and treat patients, using evidence-based care Implement and review treatment plans as appropriate Ensure interventions are undertaken in line with best practice guidance and use advanced skills appropriate to the role Ensure clinical competencies are maintained in accordance with framework for advanced clinical practice Make referrals as necessary to other members of the primary and secondary health care teams as appropriate Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency. Proactively assess and manage frail patients within their own homes to develop advance care plans to support care in the community About us In this role, you will assess diagnosis, plan, and implement programmes of evidenced-based care for primary care patients, undertaking re-evaluation and implementation of treatment plans as appropriate. In addition, you will ensure interventions are undertaken in line with best practice guidance and use advanced skills appropriate to the role, ensuring clinical competencies are maintained in accordance with framework for advanced clinical practice. You will need to hold a Postgraduate qualification within your clinical discipline, together with the certification to prescribe. You will act as an autonomous practitioner working independently and in conjunction with other healthcare professionals to assess, diagnose and treat the conditions of patients attending clinics and initiating direct referrals as appropriate. You will also lead PCN clinical projects in line with the PCN DES contract, these involve, but are not limited to, health inequalities, Learning Disabilities, Long Term conditions and Flu and Covid vaccination campaigns. You will preferably have prior experience in a Primary Care setting with the ability to make professionally autonomous decisions. You will hold clinics for patients with undifferentiated and undiagnosed problems and make an assessment of their health care needs, based on highly-developed knowledge and skills, including skills such as physical examination, applied pharmacology and evidence-based prescribing.
