Collaborative Community Care (CCC) is home to the services that lead on frailty. Be that rehabilitative, recuperation after injury or illness, assessment for long term needs or palliative care our services touch upon every aspect of frailty care. The services in the division are Outpatients and Theatres, Community Hospitals, Adult Community Therapy and Community Hospital Therapy. You will be leading a select combination of these dependent on previous experience and expertise. With LCHS recently entering into a 'Group' model with colleagues in United Lincolnshire Hospitals Trust as well as the implementation of the Lincolnshire Frailty Strategy there is significant opportunity to evolve our existing services and enrich the offer to patients. Therefore we are seeking an individual who can both deliver a Quality and Safe service in the present whilst embracing and leading change for the future. The successful candidate will be able to link the detail to the bigger picture, model exemplary professional standards and lead and inspire staff The multidisciplinary nature of CCC is reflected in the leadership team you will be joining. Applications are invited from Allied Health Professionals or Nurses. The successful candidate will hold their professional identity proudly, but lightly, because leading across professions is guaranteed in this role. This role will focus on the delivery of care in community, hospice and ward based settings managing nursing and therapy teams. It will involve integration into wider community goals and support integrated care pathways in primary and secondary care. Candidates will have experience in a leadership role within a planned or unplanned care service. The successful post holder will role model the values of LCHS, be motivated and committed to delivering our clinical strategy, and understand our role as a key partner in delivering health and social care in Lincolnshire. The successful candidate will be responsible for providing assurance against delivery, including performance management, governance, budget management, contract management, professional leadership and business planning. Utilising a range of specialist skills, partnership working and the ability to work at pace is key. As a supportive compassionate leader, you will need to be flexible and resilient to promote and maintain high performing teams. There are a number of different services in the portfolio the successful candidate could lead following discussion with the existing Clinical Services Leads.
