Main duties and responsibilities of the job: A point of contact within the practice for patients presenting with undifferentiated undiagnosed problems that require telephone triage, telephone resolution, Face to Face consultation, On-Line consultation response and home visiting. Assessment, diagnosis, treatment, prescribing, referral and discharge as appropriate and deliver key elements of Surgery initiatives. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis. Instigate invasive and non-invasive diagnostic tests or investigations, interpretation of findings and reports. Directly admits patients to secondary care outlets in medical need. Refer patients to other medical specialities for assessment where indicated including fast track referrals To take responsibility for making autonomous decisions in relation to patient care. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
