The aim will be to ensure all patients receive specialist review, as early as possible, and that resources are deployed to avoid admission or reduce length of stay. You are required to be able to demonstrate the Trust values and fulfill the responsibilities or the job description and personal specification If you are interested in the role and believe you hold the qualities we require, then apply today! You will have significant experience working effectively at Band 6/7 level in an NHS acute setting, or equivalent, working with older adults with a variety of conditions. You will have highly specialist current knowledge and skills related to the management of complex conditions within the specialty You will have a dynamic approach to work and be capable of leading a team, including in times of change. You will be expected to manage the daily running of the team to ensure the delivery of high quality patient-centred care within the specialty. This role has a strong clinical focus: You will use advanced clinical reasoning skills to provide specialist support and advice to the integrated therapy team (occupational therapy and physiotherapy) whilst maintaining a clinical caseload of complex patients as an autonomous practitioner. You will take part in the weekend and Bank Holiday rota. Work as an advanced autonomous practitioner: Assesses, sets goals, provides treatment and progresses own patient caseload which contains patients with complex and specialist needs. Lead in specialist area; contributes to development of specialist services and relevant policies helping implement policy changes and monitoring standards of clinical practice. Supervise, teach, and evaluate students and other staff working in own specialist area.
