This is a role taking advantage of the NHS Network Contract DES for Additional Roles Reimbursement Scheme. Enrolment (or completion) of the CPPE Primary Care Pathway is expected. The successful candidate will work across the 5 GP Practices in the PCN, with projects ranging from dealing with medicines reconciliation of clinical documents post-discharge, medicine safety alert processes, daily medicine-related tasks, involvement in seasonal vaccination programmes & facilitating medication reviews. We expect contribution to safe and effective prescribing across the PCN, acting on best practice from local & national guidelines. Support the role of PCN clinical pharmacists in the delivery of clinical pharmacy services to people in their own homes or local care homes.
