This is a role taking advantage of the NHS Network Contract DES for Additional Roles Reimbursement Scheme. Enrolment (or completion) of the CPPE Primary Care Pathway is expected. The successful candidate will work across the 5 GP Practices in the PCN, with projects ranging from dealing with medicines reconciliation of clinical documents post-discharge, medicine safety alert processes, daily medicine-related tasks, involvement in seasonal vaccination programmes & facilitating medication reviews. We expect contribution to safe and effective prescribing across the PCN, acting on best practice from local & national guidelines. Support the role of PCN clinical pharmacists in the delivery of clinical pharmacy services to people in their own homes or local care homes. Perform medicines improvement work to maximise safe, cost-effective best practice in prescribing Improve the use of medicines and help patients with medication concordance Assist with medication safety monitoring systems Liaise with hospital teams, clinicians and administrative staff to resolve medicine-related queries Monitor practice prescribing and adherence to practice, local and national prescribing policies Improve service and quality through audit and searches Help identify patients at risk of medicines use issues, including waste, safety and over-use About us Qualifications Essential Desirable Professional registration with the General Pharmaceutical Council Knowledge of CPPE primary Care Pathway (necessary for ARRS funding) BTEC/NVQ level 3 in Pharmacy Service Skills/Pharmaceutical Sciences Evidence of Continued Professional Development (CPD) Full Driving Licence Experience Good IT skills and literacy Experience of working with GP prescribing systems, other clinical systems, and Microsoft Office Previous experience of working in primary care, within a GP practice Knowledge & understanding Good clinical pharmacy knowledge including terminology Awareness of systems to support management of patients in primary care Knowledge and understanding of pharmacy law and ethics, and current legislation surrounding medication-use Awareness of GP budget-management and funding systems that enable Pharmacy teams to deliver local and NHS priorities An appreciation of concepts associated with primary care prescribing including rational, cost-effective and quality improvement Skills & Competencies Able to analyse and interpret data Able to analyse and interpret prescribing data Has attention to detail, working accurately and able to identify errors quickly and easily Able to manage allocated resources (time, equipment, base location) Methodical and organised approach to prioritise workload Able to think analytically Demonstrate excellent written and verbal communication skills Excellent understanding of data protection and confidentiality principles Attributes Works effectively independently and as a member of a team Flexible approach to meet service needs Self-motivated and proactive Independently mobile to be able to work across the PCN geographical area Demonstrate ability to work in busy environment, dealing with both urgent and important tasks, prioritising effectively while supporting others Excellent time keeping and prioritisation skills Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
