This area is broad and envelopes Portreath, St Day , Four lanes and Pool . We are looking for a caring, motivated and dynamic community nurse to join our large, busy, professional team. We cover several GP practices in the Redruth and surrounding area. We want you to enjoy the experience that community nursing offers and how empowering people in their own home feels so very rewarding. Your role will require close collaboration with GP's, health and social care colleagues including therapists and voluntary organisations. We would like you to possess commitment to team working and embrace innovative delivery of excellent standards of care. You will need to have a real enthusiasm for community nursing as an autonomous practitioner and a desire for ongoing professional development. You should also have an understanding of the changing climate of care delivery in the community. The ability to travel independently across the West Locality to support other teams and to fulfil the requirements of the post is essential. We work a 2 shift model to include 9 to 5 and 2 to 10 shifts which includes working weekends. Proposed interview date: to be confirmed Following an initial induction and ongoing clinical supervision, you will be responsible for: Assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment. The aim is to enable them to enjoy maximum independence and optimum quality of life.To work in partnership with patients and carers to offer support and advice and promote informed choice.To support the role of the Community Nurse Team leader.To work autonomously, providing effective, evidence-based care in line with competency and training record and organisational policies, procedures and guidelines.To support all locality teams by working in a flexible way. Moving to work with the team in most need on a daily, weekly or monthly basis as service provision demands.
