An exciting new Reactive Community Matron role has been developed in Responsive Services team in First Community Health and Care. Responsive Services comprises of Urgent Community Response (UCR), Intermediate Care Team (ICT), Integrated Discharge Team, Referral Hub and Virtual Ward. The Community Matron will work autonomously in a reactive way to prevent avoidable hospital admissions in collaboration with ICT, UCR and Virtual Ward. The ICT are an admission avoidance and facilitating discharge service, who provide assessment, short-term care and rehabilitation in the patients home. UCR are an admission avoidance team, responding within 2 hours to acutely unwell patients in the community to prevent hospital admissions. As part of the Responsive Services team you will complete comprehensive assessments in the patients home, supporting patients to manage their health and wellbeing. You will work with patients, families and community partners to support patients to manage their health needs and prevent repeated admissions. The Community Matron will work closely with the multi-disciplinary team to promote independence of patients to ensure they can remain in their own home. The candidate will be able to adapt to change and work in a busy environment. Excellent communication skills are essential to enable liaison with all our health and social care partners in the management of patients discharge from hospital and assessment in the community. You will also provide professional leadership and line management for designated staff and will have a role in the leadership of the wider multidisciplinary team. Due to the nature of the role it is essential that applicants are able to drive, have a car suitable for business purposes and have a clean driving licence.
