We are looking for enthusiastic individuals with good interpersonal skills and a passion to deliver a high standard of care to patients affected by cancer. You will be working alongside established teams of clinicians, specialist nurses, allied health professionals, and MDT and cancer care co-ordinators to provide care for patients. You will be expected to work at both of our acute hospital sites in Middlesbrough and Northallerton as required and be adaptable & flexible in your approach. The role will involve co-ordinating care from referral to treatment. You will have regular contact with patients, families, and carers to support and signpost to ensure effective progress along care pathways, in liaison with other colleagues and departments in the hospital and the community. The role will involve supporting high quality personalised care including: Carrying out holistic needs assessments and care plans Preparing treatment summaries Offering health and wellbeing advice and support Follow up for patients following completion of treatment You will be expected to attend the multi-disciplinary team meetings with the wider multi-disciplinary teams to transcribe MDT outcomes live/complete the MDT outcomes using NHS IT recording systems.
