To be responsible for the management of diabetes including insulin initiation, health education, reviewing bloods, medication monitoring and dose adjustment. Working as part of the PCN multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The role will include supporting the management team in the reviewing of clinical policy and procedure. The post holder will deliver clinic and home-based treatment plans including education and lifestyle advice, monitoring and management of symptoms and medications. The service seeks to provide access to diagnostics and early treatment intervention. The post is integral to the multidisciplinary team, working closely with our partners in secondary care to provide seamless transition for patients to primary care underpinned by a solid clinical governance structure. The post holder will build relationships with wider external organisations to promote joined up patient care and services. Key Responsibilities Responsible for the assessment, planning and implementation of clinicalmanagement plans. Deliver direct care to patients including providing personalised planned interventions to support people to remain safe, well and as independent as possible in their own homes. Initiate insulin and proactively monitor the care of all diabetic patients in line with local and national policy. Provide education and training to health care professionals across the pathway and developing the focal point for responsive, high quality, evidence based clinical advice in the out of hospital system, closely integrated with secondary care specialities. Works in partnership with other groups in the development of innovative clinical practice. Assists in managing a caseload, in line with PCN policies, guidelines and national standards. Promotes and enables patient choice and independent living. About us Service Description: West Leeds Primary Care Network (WLPCN) provides enhanced clinical services for six GP surgeries across the Pudsey and Bramley area,We have a higher than average prevalence of patients with Type II diabetes and seek to employ a Specialist Diabetic Nurse to support practices in delivering excellent diabetes care and developing a multi morbidity approach to Long term condition management West Leeds PCN currently benefits from the following clinical teams, directly employed by ourselves: Pharmacists and Pharmacy Technicians Community Wellbeing Advisers (Social Prescribers) Health Care Assistants Occupational Therapists Dietitians Learning Disability / Severe Mental Illness Care Co-ordinator Highly skilled prescribing Frailty / Home visiting team (Advanced Clinical Practitioners andAssociate Clinical Practitioners) Heart Failure Nurse Physician Associates Health & Wellbeing Coaches (weight management, older adult memory, cancer screening) The successful recruit will be expected to work across all practices in the PCN.
