The Community Pharmacy Team encompasses the Specialist Medicines Optimisation of Care Homes (SMOCH) and Medicines Support Team. This post will be fixed term for 12 months covering maternity leave. The successful candidate will have excellent clinical and communication skills and be able to lead the team of pharmacy technicians in their medicines management roles. The Community Pharmacy Team is available to patients with East Lancs-registered GPs, they undertake medicines reviews, home assessments, provide medicines advice to patients and their carers and feedback suggested changes to GPs/other health-care professionals. Perform face-to-face structured medication reviews for patients in their place of residence including nursing and residential settings with a focus on frail patients with polypharmacy and multiple co-morbidities. Produce recommendations for GPs, nurses and other members of the multidisciplinary team on prescribing and monitoring. Carry out medication reviews for after hospital admission and support transfer of care. Develop personalised pharmaceutical care plans for patients under CIC care Prioritise residents' health needs and help to co-ordinate care for any urgent problems that need attention. Work as part of a multidisciplinary team to manage medicines on transfer of care and system and liaise with other MDT members attending MDT meetings as necessary. Support staff across health and social care with queries regarding the safe effective use of medicines.
