The Community Nursing Teams in Brent are seeking Band 7 District Nurse Team Leaders responsible for providing clinical leadership in the delivery of high-quality care for the patients in the community setting and the day to day management of individuals within a team. The post holder is responsible for the day to day running of the District Nursing team to high standards within a locality. The post holder leads the team in monitoring and improving standards of care and assists the overall development of the adult nursing service. To provide managerial and clinical leadership to community nursing & Allied health professional staffs, and act as an effective role model for multidisciplinary staff members and other team members. The post holder to undertake 50 % of the time on clinical role as an expert practitioner and to provide active specialist District nursing care, maintaining clinical skills and demonstrating quality outcomes. To ensure patient safety and clinical effectiveness, providing expert evidence based clinical advice and support to all staff within the locality. To coordinate and monitor the delivery of high-quality nursing care that, where possible, meets identified patient need, supports the aims and objectives of the healthcare team, and remains within the boundaries of available resource. The post is to support the Integrated Care service manager to deliver cost effective service.
