CLCH BRENT RAPID RESPONSE - SENIOR PHYSIOTHERAPIST We are seeking an enthusiastic Specialist Physiotherapist to join our Unplanned Care team based at Sudbury Primary Care at Wembley, London delivering care to patients closer to home and preventing unnecessary hospital admission in Brent. This role will primarily work under the Rapid Response pathway, but may require the candidate to work across the other pathways when required. All pathways deliver nursing and therapy care in a patient's usual home environment. RR focuses on preventing avoidable hospital admissions for patients with acute ill-health conditions by providing holistic assessment within 2 hours of referral and treatments over a 5 day pathway. The pathway is delivered in conjunction with Adult Social Care to support the patient's reablement. The Unplanned Care team works in an integrated way with the other community providers in Brent to ensure holistic support is provided. "}
