As a registered nurse experienced in CHC, you will join a busy team of nurses who play a pivotal role in the CHC process. Knowledge and understanding of the current national and local policy/Framework Guidance relating to NHS Continuing Healthcare and NHS Funded Nursing Care is essential. Under the guidance and support of the Clinical Lead, the Nurse Assessor will assess individuals for NHS Continuing Healthcare, in line with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing care, revised July 2022. The Nurse Assessor will promote best practice and provide professional advice to Nursing Homes and other providers, promoting good relationships and liaising with colleagues from other disciplines both internally and externally to the Integrated Care Board (ICB). Assessments are completed face to face. The post holder will be required to follow due process for determining eligibility for NHS CHC for live and retrospective cases, as well as appeals and dispute cases. As part of the review process, the Nurse Assessor will review the eligibility for the NHS CHC , as well as the care plan, to ensure that all care needs are being met safely and effectively. You will monitor and influence the quality of care that is provided to patients; drawing on previous skills and knowledge to improve the patient experience of care. You will provide professional advice to patients, carers and multi-agency colleagues, including signposting to appropriate services and resources. The Nurse Assessor will contribute to the safeguarding adults from abuse process (SAFA) by undertaking reviews of individuals who have been identified as at risk of harm; as part of this process you will have a role in the monitoring and reviewing of placements/packages of care.
