The successful candidate will provide nursing care to patients in a Primary care setting, supported by a team of experienced GPs, Advanced Nurse Practitioners, Allied Health Professionals, Specialist Nurses and Practice Nurses. About us Health Assessments and Routine Screenings Conduct comprehensive health assessments for patients, including taking blood pressure, measuring vital signs, and performing basic health screenings. Chronic Disease Management Manage chronic disease clinics for conditions such as diabetes, asthma, COPD, and hypertension. Immunisations and Vaccinations Administer vaccinations and immunizations according to national guidelines and practice protocols. Health Education and Promotion Provide health education and promotion to patients on topics such as smoking cessation, weight management, nutrition, and exercise. Support patients in making lifestyle modifications to improve their health outcomes and prevent disease progression. Patient Care Coordination Coordinate care for patients with complex health needs, liaising with other healthcare professionals and community services as necessary. Documentation and Record Keeping Maintain accurate and up-to-date electronic medical records, documenting all patient interactions, assessments, and interventions.
