The post holder will provide expert clinical advice and leadership to the integrated community clinical team, drawing on clinical knowledge to undertake advanced assessment, diagnosis and treatment. The post holder will co-ordinate and manage the clinical needs of patients within the service. The post holder will lead the delivery of high quality, efficient services and service development of Urgent Community Response, Virtual Ward and the Community IV Pathway. The post holder will work closely with medical teams in primary and secondary care to deliver high standards clinical management of patients. The post holder will provide teaching and education to clinical teams.
