Come and join our friendly, sociable, valued and experienced team working within an integrated Health and Social Care community rehabilitation service based in Eastbourne, covering the Eastbourne Hailsham & Seaford area. The JCR service is a short-term service which assesses and treats people within their own home. The service aims to prevent unnecessary admissions to hospital or long-term care, enable earlier discharges from hospital and promote independence and quality of life. Previous applicants need not apply You will be part of a friendly, supportive, professional integrated team which provides a 7-day service and uses an inter-professional model of working. Continuing Professional Development is supported and supervision and training are integral to the working practices of the team. You will key work non-complex clients, provide rehabilitation to people with a variety of conditions, and respond where appropriate to urgent referrals supporting clients to stay in their own home. "}
