The Community Rehab Team are situated in Wrexham and are recognised as a vital and integral component of the Adult Mental Health service provision for Betsi Cadwaladr University Health Board. An exciting opportunity for experienced Healthcare Support Worker to work within a dynamic multi-disciplinary team, who provide Community Rehabilitation Services to the Mental Health and Learning Disability Division for the areas of North Wales. The appointed candidate will help care co-ordinators deliver a recovery focused service for service users with complex care needs within NHS rehabilitation inpatient units, out of area placements such as Independent hospitals, Low Secure Units, specialist placements and also within the community. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
