The post holder will provide advice, support, supervision, and line management to a range of practitioners, ensuring that they deliver the highest quality and most appropriate services to patients whilst driving continuous service improvement.This is an exciting opportunity for an enthusiastic and CHC experienced registered health or social care professional. We welcome AHP's, Social Workers and nurses from multiple disciplines who are looking to move into a more senior role within AACC. Our aim is to ensure individuals encounter improved experiences, transparency, and consistency across all forms of continuing care, with a smooth transition between services and resource provision.It is important that everyone receives an equal and comparable, good quality, consistent, high standard, person-centred experience.As senior specialist you will work under the guidance and support of a Principal Quality Assurance Lead to implement the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care.You will support the high-quality delivery of clinical assessments and case management, working collaboratively with key partners to achieve best outcomes.You will need excellent communication and assessment skills, be able to provide expert CHC advice and guidance to support staff involved with the assessment, planning and management of patients with complex health needs.As well as provide line-management to AACC staff, ensuring that they have the support that they require to deliver the highest quality and appropriate services to patients and to drive continuous service improvement.Successful candidates will need to have a flexible attitude and be a team player who enjoys working within a fast-paced service.Hybrid working - Mainly working from home.
