You will be responsible for co-ordinating and delivering group patient education workshops alongside the specialist nurses. You will also be responsible for providing patients with holistic assessments, giving tailored information, signposting to other services such as health and wellbeing programmes and co-ordinating their remote follow up using a follow up database system. Within this role there is scope for developing health promotion and education for patients with Urological cancers and for ensuring that patients are informed and signposted to services within primary and secondary care to enable them to live with and beyond cancer. You should possess excellent communication skills; have experience of working in a healthcare setting and ideally with cancer patients. Act as a single point of access to patients who are on self-managed follow up pathways. 2. Proactively support and coordinate for the overall needs of patients on a patient triggered follow-up pathway, using own judgement and escalating to the relevant practitioner as necessary. 3. Support and coordination of remote surveillance for patients on personalised stratified follow up (PSFU) pathways to ensure risk free survivorship. Coordination and support for the development, delivery and evaluation of tailored information and self-management education. 1. Coordinate access to personalised information and education resources for patients and their carers.
