This is a great opportunity for an ACP with diverse skills to work as a primary care ACP, managing minor illness and common GP surgery presentations plus long term conditions alongside the wider clinical team. You will need to have the ability to work independently within your professional scope of practice, to undertake patient assessment, diagnosis and clinical management within the General Practice environment, whilst working collaboratively within the practice team to meet the needs of our patients and supporting the delivery of policies and procedures. The practice prioritises education and development so is looking for a self-motivated, enthusiastic individual with a desire to learn and support others. This is a part-time vacancy for 18.5hrs per week. Please note this vacancy advert may close early if suitable candidate numbers are received. Assess and examine patients, diagnose and develop appropriate clinical management plans Assessment may be face to face, or by telephone as part of triage, and will include patients presenting with undifferentiated problems Diagnose and manage patients presenting with both acute and chronic conditions Develop clinical management plans which may include medications and other interventions Review of patient's progress and re-evaluate clinical decisions as situations evolve Assess and manage patients physiological and psychological needs including patients with complex needs, multiple diagnoses and polypharmacy Identify health priorities and make timely and appropriate interventions to support patients presenting in an emergency and/or with complex / urgent needs identify patients at risk of developing a chronic illness and develop a plan for minimising the risks of progression Advise and educate patients, promoting understanding, self-management, compliance with a treatment plan, healthy lifestyle choices and sources of support Ensure all clinically generated administration is completed in a timely manner About us PLEASE NOTE: Please see attached Job Description and Person Specification.
