The team work as an integrated team We have appointed a new Trainee Advanced Clinical Practitioner in community and there is an opportunity to develop an interest in frailty and long term conditions. You will be working with patients with a large variety of conditions and disabilities, in their own homes and local community. You will be responsible for assessment, intervention, and discharge of a caseload of patients, act as the direct supervisor to staff and provide support and expertise to the other members of the team. We offer a fabulous training programme within a supportive team The service operates across 6 days Monday- Friday 8am- 6pm, and 8.30- 4.30pm saturdays and bank holidays Part time working considered The post holder is an autonomous practitioner, responsible for provision of specialised assessment and treatment of patients referred to the service. .
