An exciting opportunity has arisen for a community rehabilitation band 6 Occupational Therapist to join a supportive and enthusiastic community therapy service, working across community delivering rehabilitation in patients own homes following discharge from an acute bed, or supporting Long term condition management The team comprises of qualified physiotherapists, and assistant practitioners, and assistant therapy staff, who work closely with the wider MDT and INT teams in community. The team work as an integrated team We have appointed a new Trainee Advanced Clinical Practitioner in community and there is an opportunity to develop an interest in frailty and long term conditions. You will be working with patients with a large variety of conditions and disabilities, in their own homes and local community. You will be responsible for assessment, intervention, and discharge of a caseload of patients, act as the direct supervisor to staff and provide support and expertise to the other members of the team. We offer a fabulous training programme within a supportive team The service operates across 6 days Monday- Friday 8am- 6pm, and 8.30- 4.30pm saturdays and bank holidays Part time working considered The post holder is an autonomous practitioner, responsible for provision of specialised assessment and treatment of patients referred to the service. . Responsible for managing a caseload of patients with a variety of complex conditions including long term condition management and acute hospital discharges. You will see patients in own homes , working with an MDT in other organisations. You will carry out specialist assessment and treatment including the prescription of essential equipment as part of the therapeutic intervention . You will play an important link with PCN and MDT teams in attending meetings as part of neighbour hood team working 6 day working is in effect - Monday- Saturday and bank holidays on a rota basis "}
