We are seeking a motivated, compassionate, and dynamic nurse with experience in care of the elderly and long-term conditions to apply for this exciting opportunity to work as a Community Matron in the One Care Home team. The One Care Home Team's purpose is to provide high-quality and safe re-active and pro-active face to face and virtual MDT support to care homes and their residents. The post holder will manage a caseload of care homes and will work in partnership with the care home team to deliver a consistent level of assessments, care and medical response to the residential and nursing homes within the borough of Barnet. We offer regular clinical supervision, guidance and training for personal and professional development in a friendly and supportive environment. This includes a program of in-service training and other training and development opportunities. Managing a caseload of care homes in Barnet. Initiating and maintaining good communication networks.
