We are looking for a passionate, skilled and highly motivated Senior Mental Health Practitioner to join the Exeter Outer Core Community Mental Health Team. S uccessful applicants will be innovative and compassionate clinicians , who are motivated to deliver individualised, person-centred care and make a positive contribution to service development. This is an opportunity to join us as we modernise and improve our services to meet the needs of Exeter and related families and carers. You will be an experienced mental health professional (Registered Nurse, Social Worker or Occupational Therapist) who has a genuine interest in collaborative working with people of varying ages and complex needs, including service users, their carers and families. You will join a rich multi-disciplinary team which includes Nurses, Occupational Therapists, Psychologists, Psychiatrists and Support Workers. We are forming closer working relationships with colleagues in Primary Care and the Voluntary Sector. We welcome you getting in touch to ask questions about the role and the service. With training and support, you will feel confident in delivering therapeutic interventions and liaising with other professionals regarding progress and outcomes. This is an excellent opportunity to learn and embed leadership and management skills. You can provide supervision as part of this role, as well as supporting the team manager with other operational management tasks. As a core member of the team, you will be responsible for your own caseload and provide assessment, case management, and deliver interventions for people with a wide range of needs. You will ensure that the service users recovery goals are central to their care plan and promote social inclusion, independence, and wellbeing. You will demonstrate excellent communication and teamwork skills and be able to work collaboratively with a range of statutory and third-sector agencies. You will have an enthusiastic approach towards your professional development.
