Burnley Community Mental Health Team are seeking to appoint a Band 5 Community Mental Health Practitioner. This post provides the opportunity for a forward thinking and experienced individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care. With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships. Over a period of 12 months, you will be expected to progress to a Band 6 practitioner. You must be a registered mental health practitioner, with excellent leadership and clinical skills. You must have some post qualification experience in clinical work with service users experiencing a range of mental health problems.You need to be able to work as part of a multidisciplinary team and have experience of planning, implementing and reviewing care needs. You will also need to have a working knowledge of the care programme approach (CPA), Dialog+, risk assessment and management, and the ability to explore issues relating to the impact of health and social needs on mental health and well-being.
