This a new and exciting opportunity which may suit someone with a formal or informal background of education delivery, supporting education delivery, counselling skills or experience in supporting the wellbeing needs of people. As Patient Educator you will learn and deliver a focused Heart Failure education and information package to patients who are newly diagnosed with heart failure and are referred into the community specialist service. You will be responsible for educating and empowering people with long-term conditions (LTC) to manage their health and wellbeing needs optimally via the use of telehealth mechanisms, which remotely monitors patients with heart failure using agreed digital technology so that they are be able to recognise symptoms early on and live as optimally and independently as possible with their LTC. As part of this role you will be expected to: Work autonomously with patients, without direct supervision and support. Deliver the approved and formatted heart failure education package initially on a one-to-one basis with patients, delivering in a variety of community settings with key carers and family present. Deliver education utilising ITvirtual platform monitoring mechanisms. To work closely and in collaboration with the clinical community heart failure specialist nursing team, GPs and practice nurses, internal colleagues within the partnership trust As heart failure education mechanisms are evaluated, innovative changes in service delivery are anticipated and future delivery could incorporate group Heart Failure education sessions With the clinical specialist team to participate in heart failure education package delivery to GPs, practice nurses, internal colleagues and other NHS trusts as part of this service as well as reviewing opportunities to further develop specialist service education delivery About us Full Job Description & Person Specification is attached: To be responsible for own time management, prioritising caseload in conjunction with service requirements. Liaise with other team members for smooth running of service by verbal or non-verbal means of communication email, written instructions, and telephone. Positively promote the cardiac rehabilitation and education programme and be involved in all aspects of Health Promotion where appropriate. Knowledge of the aetiology, pathophysiology, epidemiology, diagnosis and management of cardiac disease and their related comorbidities. Undertake study and specific training as required to fulfil the role. Maintain knowledge of local, national and NICE Guidance for Heart failure conditions. It will be the post holders responsibility to provide updates/changes to processes relevant to the education of Heart Failure patients. Ability to demonstrate the dexterity, co-ordination and sensory skills required to take psychosocial observations, following appropriate training.
