We are looking for an experienced Mental Health Practitioner: nurses/social workers/ and occupational therapist to work in the PCN as part of the Additional Roles Reimbursement Scheme (ARRS). It is an exciting time to join services in Kent as we currently undertaking a programme of transformation of community Mental Health services, and these roles are the building blocks for those developments. You will primarily work in the PCN as a member of the integrated team in GP surgeries. The practitioner will be embedded as part of the PCN Multi-Disciplinary Team. Working alongside the other roles, they will also have close links with Kent and Medway Mental Health Trust services. The practitioner will provide a \"bridge\" between these services, delivering high quality mental health care in Primary Care Settings. The role requires a highly competent, proficient practitioner, who is able to work autonomously & creatively in this busy demanding role. Excellent assessment skills and knowledge of local resources is essential to this role, as it is the gate keeper and guardian for referral to secondary care services. We are seeking experienced Mental Health Practitioners who can come and make a difference.
