If you: o are a qualified RMN, Social Worker, or Occupational Therapist with a flexible approach and want to work in a team that encourages new ideas and feedback on how to improve the service; o share our values and are committed to providing high quality care and developing you own skills; o have confidence in dealing with a wide range of situations and other professionals to provide the best care to people within the prison; o are enthusiastic about working within an ever-changing prison environment; o would like to work within a fully multi-disciplinary team to provide varied and comprehensive interventions in-line with the stepped care model. You will be responsible for the assessment, planning, implementation and evaluation of evidenced based interventions, managing a caseload, practicing within the framework of the multi-disciplinary team and Care Programme Approach. You will provide a care coordinator role and communicate effectively with other professionals, internal and external to the Inclusion team and prison, to ensure through care support where appropriate. You will work with individuals from initial assessment through to discharge, which can include transferring care to other providers such as hospitals or CMHTs whilst working with a variety of agencies in the Offender Health Pathway.
