About us Clinical Role Function as a key member of the clinical team at St Marys, delivering primary care services to patients. Deliver a high standard of patient care as an Advanced Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base. To deliver clinical assessments and primary care services for patients of St Marys surgery, both remotely (over telephone) and face-to-face, including in the surgery and in the patients own home. Provide scheduled (booked) care and on-day assessment for patients presenting to the practice with more urgent needs. To undertake assessments as required in patients own home and other community settings as appropriate. Make professionally autonomous decisions for which the post holder is accountable. Manage an undifferentiated caseload of patients presenting with acute and chronic, complex health needs. Contribute to the management of care for patients within a list based primary care model, contributing to an MDT/Clinical firm to promote continuity of care. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports. Provide safe, evidence-based, cost-effective, individualised patient care.
