They will work within the multidisciplinary team providing support, dealing with non-complex tasks to allow registered practitioners to focus their expertise on managing patients complex care needs. They will promote self-management and provide coordination of care, utilising a person centred approach from diagnosis, through treatment and beyond. The Assistant Practitioner will carry out interventions within agreed parameters and defined protocols of care under the delegation of registered nurses within the speciality. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Practice in accordance with Health Board standards and statutory requirements, operate within the boundaries of the role, and assessed competencies. Work proactively as a member of the multi-disciplinary and multi professional healthcare team. Manage a defined caseload of patients under the supervision of the CCNS. To carry out assessments and interventions as directed by the CCNS. Assist in identifying health needs of patients within scope of competence. Encompassing planning, organising and delivering clinical care as agreed with the CCNS, in line with clinical governance and within agreed professional standards and guidelines. Undertake reviews and evaluation of the care provided under the indirect supervision of a Registered Nurse and make suggestions to the registered nurse in relation to adjustments to the patients care plan. Discuss and make suggestions for adjustments to individual care plans and their health and well-being needs with the care team and understand own role in delivering care to meet those needs.
