This post hasbeen covered temporarily for the past 3 years by the Associate Specialist in the team 'ac?ng up'. The post holder will carry no responsibility for inpatients but will be part of the bed management strategy within the Community Mental Health (CMH) Directorate. The Directorate takes a whole system approach to bed management with twice daily meetings between community and inpatientclinicians to ensure that beds are used in the most efficient manner. There is additional community support in the form of our admission avoidance team, the home treatment team and our crisis housebefore patients may need a hospital bed. The ADAPT Pathway provides focused, therapeutic interventions to working age adults residing withinthe Greenwich Borough who require care and treatment for anxiety, depression, affective disorders(without psychosis), personality disorders and trauma.The service aims to be flexible and accessible. The pathways of care aim to offer individuals, choiceand involvement regarding their treatment, actively encouraging their participate on in the planningand implementation of their care. A personalised care plan will be devised in consultation with theservice user and their Carer/family where appropriate.The Greenwich ADAPT Team was formed as part of a reconfiguration of Community Mental HealthServices that occurred in September 2015. The reconfiguration of Community Services was part ofthe directorate's plan to ensure efficient delivery of mental health services whilst delivering highquality care. About us The ADAPT Pathway provides focused, therapeutic interventions to adults residing within the Borough who require care and treatment for anxiety, depression, affective disorders (without psychosis), personality disorders and trauma. The service aims to be flexible and accessible. The pathways of care aim to offer individuals, choice and involvement regarding their treatment, actively encouraging their participation in the planning and implementation of their care. A personalised care plan will be devised in consultation with the service user and their Carer/family where appropriate. Whilst Pathways of Care utilises a team approach to the delivery of care, every individual receiving care will be allocated either a health care professional (HCP) or care coordinator. This worker will be responsible for ensuring that care is delivered in accordance with the care/treatment plan, and that the needs of the individual are regularly reviewed, risks and care plans modified accordingly.
