This Senior Community LD Nursing post will cover maternity leave for 1 year. The post will be within the Complex Physical Health nursing team, and will incorporate the following: To provide visible, high calibre nurse leadership and support around the 'Complex Physical Health' (CPH) of Learning Disability within a multi-disciplinary framework. The post holder will lead the development of complex physical health of Learning Disability practice, promoting the clinical and professional development of staff within the community team, to ensure they provide holistic and evidence based care. The post holder will provide specialist assessment, intervention and management, working collaboratively with colleagues, service users, families and other agencies. The post holder will lead on providing robust health liaison to acute and primary care to ensure our service users are able to access mainstream health services. 5.
