You will work within the multi-disciplinary team and provide Community nursing support to adults with Learning Disabilities. You will also work with all stakeholders, to contribute to learning and development of staff working with adults with Learning Disabilities, ensuring effective service delivery and the best possible outcomes for our service users You must have up to date knowledge and appropriate skills which will ensure that Community Nursing interventions are delivered in the way which meets the needs of the service user. You will have an understanding of the complex needs of people with learning disabilities and be able to demonstrate this with recent and on-going clinical professional development. You must have excellent communication and organisation skills. You must also have experience of working within a Community Learning Disabilities Team. You will manage a case load and supervise junior staff with a delegated caseload and support them in their development. You will be a car driver and have access to a vehicle to meet the requirements of this post. To provide an assessment and therapeutic service to Adults with a Learning Disability who have a range of complex needs in a variety of settings including day services, residential settings and the persons own home; assessing, prioritising, formulating, planning, treating and discharging patients autonomously. To work as a member of the Community Learning Disabilities Team, contributing to multi-disciplinary working as necessary, to address the needs of clients referred to the team and manage a delegated caseload. To work collaboratively with other specialist services within Learning Disabilities to meet the needs of clients. Be regularly involved in supporting and consulting on complex transitions e.g. to new home environments; leaving school; admission and discharge from inpatient services. To participate in Service Developments, Audit and R&D activities as directed by the Appropriate Lead. To actively participate in a programme of supervision, competency development and CPD activities under the guidance of senior clinicians. To work within guidance and framework for Safeguarding Adults / Child Protection Policies. To provide verbal / written reports in respect of individual clients. Work within clinical governance framework. To promote a healthy lifestyle within the client population. Attend multi-disciplinary reviews and CPAs. Act as care coordinator.
