The newly developing strand will offer advice and support for people who may be having some unusual experiences that are causing them distress, or may be experiencing psychological problems. We aim to help people get their life back on track. Sometimes people need a bit of extra help. The post holder will provide a range of services including practical and emotional support to individual service users as indicated by the individual care plans provided by the care team. Under regular supervision the post holder will be participate in the assessment, monitoring and interventions of care needs for individuals between the ages of 14 and 64 years within the Early Intervention in Psychosis service. This vacancy will be covering Mid Cornwall working inline with the area model of care. Establish and maintain effect contact with service users, their relatives and carers in order toensure good communication. Give information and assistance to service users and their carers so that they are aware of theirrights, entitlements from the team and wider health and social care community. Work with service users and their carers to access identified services. Monitor mental health and be aware of fluctuations in mental health through observationsand report changes to the team practitioners so that service users are directed to appropriateservices and risk is managed.
