Ideally applicants would be able to start as soon as possible, but later and earlier start dates would be considered. The appointee will provide both inpatient and outpatient care of general respiratory and general internal medicine patients; supervise care of the patients on the medical High Dependency Unit 1 week in 7, as well as supporting the team with the bronchoscopy lists and pleural procedures. The job is a full-time post comprising of ten programmed activities, with supervision of the junior doctors and specialist nurses. There will also be an on-call commitment to cover the general medical weekday rota (1pm to 9pm and non-resident overnight) on a 1 in 20 basis and respiratory weekend days (8am - 5pm) across FHFT. We are seeking a Consultant Respiratory Physician. This is a full-time appointment with a commitment to the general weekday medical rota (approx 1:14) and the respiratory weekend rota 1 in 8. The appointee would be expected to undertake on average: 2 general respiratory outpatient clinics in clinic weeks 2 specialist clinic - TB/CF/ILD /Sleep/Cancer Weekly procedures list (bronchoscopy, EBUS or pleural procedures depending on experience / competence) 1 in 7 week ward commitment 1 in 7 week respiratory HDU commitment Triage of ERS referrals About us The job also entails inpatient care as part of the respiratory megafirm, looking after approximately 50 inpatients with other colleagues. The consultants currently rotate on and off the wards, doing a ward block and weeks on HDU. During the ward block (respiratory ward and the HDU); we provide daily consultant input, in-reach into the medical assessment unit and emergency department and the provision of inpatient consultations. The respiratory megafirm has 3 SpRs, 2 IMT 3s 7 SHOs and 3 FY1s. There is a dedicated 16 bedded medical high-dependency and NIV unit, where patients are under the care of the respiratory team. There is also a general ICU which is run as a closed unit, with specialist input as required. There is a dedicated medical day-unit for out-patient pleural procedures, line insertion, day-case biopsies, commencement on intravenous antibiotics and out-patient support for patients with long-term chest drains. The specialist nurses (cancer, TB, non-invasive ventilation and sleep, airways disease and ILD) participate in specialist out-patient clinics, review in-patients and assist with discharge planning. There are also community respiratory care teams that work in both the in-patient and out-patent settings covering Windsor, maidenhead and Berkshire. Approximately 200 bronchoscopies are carried out per year and 200 endobronchial ultrasound procedures. We also carry out thoracic ultrasound and provide sub-regional support for tunnelled chest drain placement. We have an ambulatory care pathway for management of pleural effusions and have 2-3 pleural disease clinics a week to support this. The lung cancer multidisciplinary team comprises chest physicians, a radiology team, an oncologist based at Mount Vernon Hospital, but who run clinics and oversee chemotherapy at Wexham Park, and Thoracic surgical team based at Harefield and & The Royal Brompton Hospital.
