The post holder will deliver high quality comprehensive mental health services. The principal purpose of the job is to improve the psychological health, wellbeing and social functioning of people with complex psychosis within an identified Devon locality. The post holder will join a place based Recovery and Rehabilitation Service delivering recovery focused, evidence lead interventions and support for individuals with complex psychosis, their families and carers. The post holder will operate flexibly across the domains of the service (intensive inpatient, intensive community deliver, ongoing community delivery and wider place based development). The post holder will work as part of a specialist rehabilitation service working with a clinical caseload of individuals with complex psychosis, using specialist, social work and social care focussed evidence based assessment in line with the principles of the Care Act 2014. The post holder will act as a lead specialist for the Recovery and Rehabilitation service, providing professional clinical supervision to less experienced staff, trainees & students, and contributing to the leadership of the local service. The role involves managing a defined caseload of individuals with a focus on providing expert assessment and treatment for people with a complex mental health diagnose. This will include recovery co-ordination (design and co-ordination of treatment packages that are personalised). The post holder will provide statutory Adult Social Care, Care Management functions (assessment, care planning and reviewing) and liaison with a wide range of statutory and non-statutory agencies is central to this role. The post holder will contribute to the clinical leadership of the team, providing supervision for other members and participating in team and service development, and evaluation. As an inpatient and community Rehabilitation Social Worker you will assess and plan support for a range of complex case work, as well as providing professional advice, mentoring and support to your colleagues, newly qualified social workers and student social workers. We have an expectation that you will have a good working knowledge of social care legislation (The Care Act 2014 and its interface with S117 MHA 1983/2007). As part of the MDT you will undertake safeguarding enquiries where adults are at risk of abuse or neglect and with sound assessment skills, undertake Mental Capacity Assessments that inform Best Interest decisions to ensure that people are living as independently as possible. Employing a strengths based approach, you will be creative in your thinking and employ a person-centred approach to your work. Good time management and organisational skills will support you to manage your work/life balance. We are looking for enthusiastic Social Work professionals across Devon who can evidence a commitment to strengths-based practice, a real pride in their profession and passion for delivering Social Work and Social Care interventions as part of a varied and skilled MDT. You will be an experienced and qualified Social Worker (CQSW, DipSW, BA Hons, MA or equivalent) who is registered to practice with Social Work England. You will have completed the Assessed First Year in (post-qualifying) Social Work practice, if appropriate, and can demonstrate a commitment to ongoing professional development as we are committed to supporting you to look at post-qualifying opportunities during your career with us. Bringing a comprehensive working knowledge of the issues currently impacting on social care delivery, you will be up-to-date with policy and practice development and be confident in your knowledge base and application of the Care Act 2014.
