We are looking to recruit a Band 7 Diabetes Nurse Specialists who is passionate about diabetes care. We are a multidisciplinary team consisting of consultants, dieticians, podiatrists as well as specialist nurses. The team receives referrals from many primary care services as well as local acute hospitals. To work as an independent practitioner and demonstrate specialist skills and knowledge through expert nursing advanced clinical skills and maintain a high professional standard of care and expertise, acting as a role model for other healthcare professionals. Work with Diabetes Service Lead and Consultant Clinical Lead, assess referrals, prioritise, and decide when patients need to be seen and by whom (i.e Consultants and Nurses), and refer patients to other services (Podiatry, Dietetics etc). To hold a caseload of patients with type 2 diabetes, undertake home visits for the assessment and review of housebound patients with diabetes. Provide support and education to GPs and practice nurses and make recommendations relating to the adjustment of medication and treatment and the need for diagnostic tests. To work with a range of agencies and health care professionals including District Nursing, learning disabilities and nursing homes and effectively communicate information to ensure continuity of care. The post holder will provide support to primary care practice and other services to develop a structured approach to the management of Diabetes and work to agreed clinical protocols and guidelines ensuring comprehensive care across the diverse population of the Borough. About us You will be an experienced Diabetes Nurse with relevant post registration training. Ideally be an independent Nurse Prescriber or demonstrate a willingness to achieve qualification. You will have supervisory responsibility and able to guide and teach members of the clinical team.
