This post is aligned to Sandy Lane Surgery and Central Park Surgery but you may be required to work at any one of our member practices. This is an exciting opportunity for an experienced paramedic to work in general practice seeing patients in surgery and carrying out home visits for housebound and care home patients across the Leyland PCN footprint. The candidate should be comfortable working independently as well as part of a wider multidisciplinary team. You will require excellent leadership skills and will play an important role in the development of the service. You will participate in, and influence efforts, across health and social care to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through proactive provision of care in or as close to the patients own home as possible. The PCN paramedic practitioner will actively support integrated neighbourhood ways of working to support standardisation for efficiency purposes. Paramedics work autonomously within the community using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains and headaches. They will demonstrate safe, clinic decision-making and expert care, including assessment and diagnostic skills, for patients within their own homes. Along with using a medical model to assess, diagnose and prescribe for acute medical conditions, the clinician will use a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team. About us The core responsibilities of our PCN paramedic practitioner include the following. They will: 1. Monitor, review and evaluate care interventions to ensure optimum quality is maintained 2. Lead on the assessment, diagnosis, planning, implementation and evaluation of treatment/interventions and care for patients presenting with an undifferentiated diagnosis. 3. Clinically examine and assess patient needs from a physiological and psychological perspective, planning clinical care accordingly. 4. Assess, diagnose, plan, implement and evaluate interventions/ treatments for patients with complex needs. 5. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). 6. Diagnose and manage both acute and chronic conditions, integrating both drug and non - drug based treatment methods into a management plan 7. Manage care of patient at End of Life and ensuring palliative care plans are put in place and actioned as necessary 8. Undertake and manage remote consultations 9. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice 10. To provide assessment, treatment and diagnosis at point of first contact by attending to patients in a variety of clinical or non-clinical settings according to patients needs. 11. Work with patients in order to support compliance with and adherence to prescribed treatments. 12. Provide information and advice on prescribed or over the counter medication on medication regimens, side effects and interactions. 13.
