An opportunity has arisen within NHS North East Commissioning Support (NECS) to join the All-Age Continuing Care team (AACC) as a Band 6 Continuing Health Care Clinician, to deliver an effective assessment, review, and case management service in the Durham Tees Valley area. We are looking for candidates with professional registration from a range of backgrounds who can evidence how their qualifications, skills and experience will contribute to NECS values and strategic goals. The successful applicant will demonstrate knowledge and skills in completion of person-centred assessments to aid decision making for NHS funded care, working within a multi-agency environment Ensuring that health care needs of clients referred for continuing health care (CHC) eligibility are identified and met in accordance with the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care (FNC) Collating and undertaking comprehensive and co-ordinated assessments for decision making for NHS funded care with clients, carers and colleagues within health and social care and care plan and commission services to meet the needs of eligible clients Case managing CHC clients ensuring timely reviews are conducted to ensure health and nursing care needs are being met appropriately and to ascertain care packages continue to be effectively commissioned and are cost effective in line with National Policy Promoting Personal Health Budgets (PHBs) ensuring integration into continuing healthcare and that they are offered where requested to those clients who meet agreed criteria Role requirements: Registered Nurse qualification, AHP or Social Work qualification Relevant experience of working in a complex organisation NHS or Social Care Experience of working in partnership with the wider health and social care team Experience of working with vulnerable people and their families/carers in the community and in acute settings Demonstrate ability to work to defined targets and deadlines About us Ensure that the health care needs of clients referred for continuing health care (CHC) eligibility are identified and met in accordance with the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care (FNC). Collate and undertake comprehensive and co-ordinated assessments for decision making for NHS funded care with clients, carers and colleagues within health and social care and care plan and commission services to meet the needs of eligible clients. To promote Personal Health Budgets (PHBs) ensuring integration into continuing healthcare and that they are offered where requested to those clients who meet agreed criteria. Attend the panels where arranged to present own cases and provide clinical information. Case manage CHC clients ensuring timely reviews are conducted to ensure health and nursing care needs are being met appropriately and to ascertain care packages continue to be effectively commissioned and are cost effective. Assist in maintaining a database for all patients assessed for continuing healthcare / funded nursing care and ensure proper quality assurance systems are in place. Develop strong working relationships with partner agencies including Acute Hospital Trusts, TEWV, local authorities and the Independent Sector. Provide professional nursing advice to Acute Hospital Trusts, TEWV, nursing homes and social services with regards to Continuing Healthcare and Funded Nursing Care. Work with the continuing care team, ensuring that the reviews are carried out in a timely manner including any formal reviews that may need to be referred to the Continuing Care Manger.
