We are seeking enthusiastic Patient Care Navigators to join our dynamic and friendly team. In this role, you'll play a crucial part to the delivery of care for our patients. Providing acute care and continuity of Care. With very light Admin duties and a large supportive team. We use EMIS, Docman and AskMyGP. Our patients value the service we provide and we have 4.7 on Google with over 800 reviews. We are very light regarding home visits or practice admin. Minimal medications, bloods or document processing. We have a wide MDT with multiple specialisms in house including dermoscopy and minor surgery.
