To provide and maintain a high standard of chronic disease management for patients with chronic diseases, which includes the diagnosis, monitoring and development of individual management plans, and agreeing these as appropriate with the patient and other health professionals To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team.
