The successful individual will be based at Halesfield, Telford. They will join an enthusiastic and supportive multi-disciplinary team in providing high quality care and rehabilitation. The successful individual must have an interest and experience in patient rehabilitation with in the Community setting and Bed Based Rehab in care home setting and be able to work as an autonomous practitioner. To provide a key role in facilitating the seamless transition from community hospital to patient's own home and transferring the care of the patient to other appropriate services to maximise the patient's independence and quality of life. To provide supervision, facilitate the personal and professional development of junior members of staff, enhancing their skills and knowledge and where appropriate line management.
