The Occupational Therapy Service in Community Hospitals aims to develop the patient's optimum level of function and independence in the physical, psychological and social aspects of life, facilitating timely, safe and sustainable discharge. The focus of this role will be to plan and coordinate the safe discharge home for adults with a range of complex needs, liaising with community services to support a discharge to assess philosophy.
